To file a claim, you'll just need a few key details to help Fortegra verify your device and coverage.

  • Phone number or email address associated with the account.
  • The device's IMEI (International Mobile Equipment Identity) number, typically found under Settings > About Phone.
  • A copy of the policy or contract number (or equivalent identifier) so the claim can be linked to the correct coverage.
  • If applicable: documentation around the incident to help support review during claim processing.
  • Payment of the administrative fee which may be refundable if the claim is not approved.
